All services are coordinated by knowledgeable and empathetic Case Management Teams, which include a Physicians, Registered Nurses, Social Workers, and of course Case Managers.
Post-acute care, skilled nursing care, wound management, and general nursing services are provided by Registered Nurses, and Licensed Practical Nurses.
Rehabilitation services available include Physical, Occupational, Speech and Respiratory Therapies. These services are individualized to serve the specific needs of each resident.
Therapeutic Recreational Departments provide a complete schedule of social events , arts and crafts, education, cultural discussions, current events and of course varied and stimulating entertainment.
NuVision Management client facilities offer a complete continuum of care through their on-site Subacute Rehabilitation Programs. The subacute rehabilitation program approach centers on a multi-disciplinary team of experienced, board certified physicians, nurses, social workers, dieticians, physical, occupational and speech therapists. The team works with the patient on his/her medical, social and emotional needs through an individualized care plan. The goal is to help the patient reach the highest level of function prior to returning home.
